New Patients 
New patients who live within the Practice boundaries are welcome. In general, this covers an area within a 6 mile radius of the Practice. Please see the map.
Please note that you no longer register with a particular GP in the Practice, but with the Practice itself.
To register, please bring your medical card to Reception. If you do not have a medical card a HS200 form will be given to you for completion, providing you have continuously lived within the United Kingdom. If you have ever lived outside the UK (this includes ROI) you will be asked to completed a HS22X form.
All new patients registering with the Practice are required to produce photographic ID, passport, work permit or visa.
We ask all new patients to attend for a new patient health check within four weeks, and you will be asked to make an appointment when you register.
Where patients are requesting to join the Practice list, the Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
